We were looking for a form builder that integrates nicely with our branding, and SK more than delivers. The app is very easy to set up and customize, and includes many block elements for different data such as free text, radio buttons, star ratings, check boxes, etc.

As a big plus, you can upload and submit files, and set conditional logic to create workflows depending on the answers on your form. You get email notifications when a form is submitted, but can also retrieve all submissions from the App admin.

The support team is very responsive, and was quick to jump in and assist us during the implementation.
