Just installed this app and so far its been great. Easy to use and has all of the features some of the expensive apps have.

Recently setup to be used as a Returns Form and works great with easy implementation into both the shopify store and our current internal RMA process. Considering it is Free I can happily say it is the ideal option for Small to Medium Business where a quick form is all you need.

After searching lot of forms i found this app. It offers exactly same features which I required to run my website. The user interface of form makes it incredibly easy to use, even for someone like me who isn't tech-savvy. Highly recommend!
