It's very easy to use and it's a great solution to create a form in the website. Great for all kinds of custom forms.

Easy to integrate and to build a form. I just have a bit of an issue with the tagging but overall experience is great. customer support is responsive.

Recently setup to be used as a Returns Form and works great with easy implementation into both the shopify store and our current internal RMA process. Considering it is Free I can happily say it is the ideal option for Small to Medium Business where a quick form is all you need.
