This app is fantastic! It perfectly meets my needs, and I highly recommend it!
It's quick and so easy to use! It literally took me 15-20 minutes to set up the form and load it onto the site.
So easy to use and customize... the prompts weren't exactly what I needed but I was able to change it to my needs within 5 minutes super easy!!

Awesome app, great that it's free
SK's Forms is simple and easy to use and perfectly meets my requirements. I have used other form apps before, but the experience is very poor, such as not being able to receive email notifications, not being able to open the app, etc. But SK's form avoids these problems perfectly. Highly recommended!
