Fantastic app and very cheap too but ace quality.
I use it instead of the bog standard contact page as it has spam guards. I have also customised it to suit my brand. Love it!

It amazes me that Shopify does not have contact forms built-in as an option... But this one is a super easy contact form app. We installed in just a few minutes on our men's gift shop a few months back and have been very pleased.
