I really like this app, i have used it on my site that sells awesome hoverboards. The app has a very slick hover drop down effect and a cool lightbox. Love the ability to add a custom skin background. It has definitely added a little sparkle to the normally boring contact forms that you get. 5 out of 5. Thanks for this excellent free app, hope it stays that way.

I would like to suggest some improvements:
1) opportunity to insert conversion tracking tag on "send" button
2) creating active phone call button, so visitor could click on it and started a call.
