Works great. Just wish the pricing was slightly better since I only need 2 simple forms but want them embedded on the page (which requires the paid plan).

Quick to set up and easy to use. We used it as a pop up survey form for our customers so we could gather improvements they wanted for our website

You could embed our Contact Form to your page. That feature is available in Premium plan. Our form will be embedded to your page instead of being popup.

I used this app to create a popup contact us form. Very nice formatting, I like the pictures. I'll see if I can personalize it a little more...
