Really easy to use and customize...only took a few minutes. Customize fields, etc, even add an image to form. Works as described. Thank you!

Great App. Great usability. And Super fast first level support! Can recommend. I just had a quick error, that the form wasn't displayed and the support was able to give me direct and fast (less than one minute) reply with instructions + video. And that worked pretty well!

I am NOT tech savvy at all, but I managed to install app with a little patience, and from viewing the two videos very carefully. Have a little refining to do. Waiting on response from tech support.
