Amazing - simple, super quick. Would recommend
We used this app for many aspects of our website and it has helped us tremendously. The customer service is very knowledgeable in all areas of shopify and coding/integrations and they are very responsive. In addition to the using the app for custom signup fields, We actually use it as a central database for storing crucial information on all our customers, including using it as an infrastructure for storing back end data on each customer in an organized fashion. We highly recommend this app.

All I can say is try it, you WILL like it. If you need complex customer fields (serial numbers, date of purchase, toggle entry, image upload, etc.) for your business this is the best out there for Shopify

This app allowed us to create a custom drop down menu on our sign up form and customer profile page. Easy to use, easy to configure.
This is the only app that allows my customers to upload a document and send it right to me via email. Great app. Great Customer service.
