I used this app to create a custom form for customer product registration. Simple and straightforward process and our custom form works well. Their customer service responds quickly and took care of slight adjustments to our form quickly.

I like the app but don't like the constant pop up to give it a rating after telling asking not to show again.

The team is awesome. Form wasn't built to accept PDFs so I emailed in and they built the functionality overnight. Really cool. Great customer support.

Cheap and easy but missing a critical component of form building, which is to move questions around. Can't use it because of that as going back and making edits to your questionnaires is too time consuming to re-do.
