App of great use and customer support easy to reach ! They helped me very quickly and were very professional.

I used this app to create a questionnaire and needed support to complete the task. We received excellent, follow-up support. Everything was done promptly. Very appreciative of the fine work.

The free version lets you create a form, but does not allow you to see the results when the form is completed. You have to upgrade, which of course makes the "free" plan useless.

First class customer service! I installed the app (which was very easy) then I needed help with getting my questionnaire up and running so I contacted them and they responded and got things going right away, no hassle, no drama. I am very pleased with this app.

Would be nice to have more controls and default text.

The app was so easy to use and allowed me to customize the exact fields I needed. The perfect solution for my shop!
