I havent gotten a form request in minth although ppl have been filling it out. I dont recommend this app. Customers are filling out the form and i have no knowledge of it
Great app if you want to add forms directly to pages instead of templates or run translations ... but the forms load quite slowly if embedded directly on a page (our use case)

Hulk Form Builder has been really helpful for our Shopify store. The app provides good flexibility for creating and customizing forms, but what stands out most is their quick and responsive support.

Excellent support. The necessary forms were transferred between our store instances in just a few minutes, so we didn't have to create them from scratch.

The form stopped working on Google Chrome for over a week, which caused a number of customer complaints on our end. That said, once we raised it, customer service responded well: they offered a discount as a commercial gesture and helped us work through a solution. We appreciated the support.

The staff know absolutely what they're talking about, even on the advanced things like amending features with JavaScript to make your forms extra functional. For us, this was far superior to alternatives to get requests for quotes.

Great form with responsive support. Hopefully load time is improved. The emails requesting to close a ticket can be less, as they can be exhausting. Great app, generally.

I used Hulk Form Builder to create a registration form in Shopify for our Team Competition. The setup required a more advanced workflow, and when I was unable to achieve the desired result on my own, the customer support team was very helpful and responsive.
The developers went the extra mile and helped implement a solution where the product metafield value is included in a hidden field within the form. This worked perfectly and solved our registration workflow needs.
