I've just set up my first form with the Hulk Forms app, and I have to say it's awesome! I tried it as I needed a form where my customers could upload images (how is that not a built-in feature of Shopify?!) and I've been blown away with the level of options. Whatever your use case, this app can definitely handle (but you may need to be at a paid plan for some of the customizations you need).

The only feedback I have is: I wish you could make a section conditional on multiple options (so if they selected option 1, 2, or 4, show the image upload button). Luckily you can easily create multiple conditional sections, so it's easy to accomplish, but it would help keep the forms as clean as possible - rather than having duplicate sections.

The Hulk form builder is very easy and intuitive to use. It creates a super clean looking and very functional form for almost any kind of form you would want, I'm using it for a brand ambassador application and it had every feature I was looking for and more. I needed some assistance with the app integration to my theme so I opened their live chat and within an hour I had told them what I needed help with, they sent clear directions on how to do it myself, offered to do it for me for free, and when I accepted their offer to just do it for me, they had it done within a matter of minutes while being super friendly and professional the entire time! I will be working with them again and recommend it for others as well!

I initially had trouble connecting to Zapier (confusion on my part), but tech support was outstanding and clarified the issue very quickly! Tarik in particular was incredibly helpful.

I quickly had my new form (a "Get Free Sample" form) up and running live and connected to Zapier for additional automation flow (create new order in my shipping software). The form builder itself is intuitive and flexible. I particularly like the ability to create new Shopify customers when the form is submitted (optional feature you can toggle on/off) and to also limit the form submission to one per email address (can also toggle on/off). There are lots of additional features that I am looking forward to using.

I've tested numerous form build apps and Hulk Form Builder is a great product and the tech support is excellent! I can highly recommend this app with confidence.

Outstanding app! It does exactly what I expected and more.
It turns out it wasn't the right tool for the job in my case, which was trying to create a product recommendation quiz for Shopify. However, it's a fantastic feature-rich contact form, and will definitely use this in the future. I was thrilled to see how many awesome features were given even with the free service.
