Have been using this app for years, along with other apps by the developer. Love all of them. Recently ran into an issue where my forms weren't loading on my website. Works great, its a simple and easy way to get form uploads in Shopify without having to do your own complex setup. I had an technical issue, but there support responded in 2 minutes and fixed it right away.
