I needed a form to collect information for device activations. After searching and trying a few I settled on Hulk Form Builder. I found it very easy to use, and the customization is amazing. Adding the completed form to your page is super easy as the app does it for you. Make sure to enable the app in the App embeds section of your site other wise your form wont show. I reached out to support and they quickly helped me out with this.

Great app and a good quality customer service,I received a support from Adis, he was so helpful and kind.
I have installed the Hulk form builder app earlier this week and ran into some issues getting it to appear live. I was amazingly pleased that they immediately replied after my support request and after a few emails, we resolved the issue. Muaza was not only patient and helpful but also a real pro who knew his stuff.

Only had the app installed for the best part of 24 hours, however so far so good. Seems to work for our needs, however time will tell. Support was very useful and very quick to help when needed.
