Did not find anything free on this app as advertised. Every button I pressed to create a form tells you to upgrade to use that feature. Not impressed.

You don't have to go for PRO if you don't need any advanced personalization. Let us know if you need any help in creating a form, our team will gladly take care of your needs.
Free version is soooo limited it basically won't let you do anything.

Sorry that you feel that way. We respectfully disagree with your review as it's not accurate and not fair. We offer Unlimited Forms and Fields under the free version. Yes, there are some paid versions but over 8000+ free customers use the form daily. If you have issues with our app from a functional or support perspective, feel free to share that but it's not fair to add a negative review because we want to charge for features we've built and make sure our business makes money. I would kindly request that you contact our support team and delete this review. If you have any issues with the form builder app, we are happy to help resolving any issues you are facing.

UPDATE from ~6 months in: Still 5/5 stars. I've reached out to support for the odd question here and there, and every time I've gotten a response within *minutes*. And it's not just an auto-responder. It's a knowledgable, helpful person every time. Can't say enough good things about this app.
Original review: I've just set up my first form with the Hulk Forms app, and I have to say it's awesome! I tried it as I needed a form where my customers could upload images (how is that not a built-in feature of Shopify?!) and I've been blown away with the level of options. Whatever your use case, this app can definitely handle (but you may need to be at a paid plan for some of the customizations you need).
The only feedback I have is: I wish you could make a section conditional on multiple options (so if they selected option 1, 2, or 4, show the image upload button). Luckily you can easily create multiple conditional sections, so it's easy to accomplish, but it would help keep the forms as clean as possible - rather than having duplicate sections.

I used this app to create a "Get Involved" form. It's free, and it works great. My favourite feature is that you can have some questions only appear on the form if the visitor selects a specific option from a drop down menu. Also, you can have an automatic response sent to the customer from your email address when they fill out the form - so they know that the form went through. No complaints here - great options for a free app.
