Works beautifully! I needed to add a lot of custom fields to the form. I have tested it a few times and edited it over and over, which was really easy.
Just a little slow to load, but I have some text on the page also which loads first, so it is fine!

This tool even free features are very powerful, very meet my needs, both can customize the form, and can export the submitted form information, in short, very easy to use!

Excellent customer service. I had an issue that was caused by something I was doing wrong and they helped me resolve it immediately at 10pm at night! I highly recommend this app!
