First off, I love this form app. Even the free version has everything that I need and their customer service is off the hook! They are quick to respond and are more than happy to help. I have a theme that allowed my main banner to appear on the front page of my site and my contact page only. I didn't know how to add it to any other pages. I wanted to use my Hulk form as my contact page, but I couldn't figure out how to get the banner from the built in contact page to show on my form page. But, Andres made it happen! I'm so happy and thankful for his help! Awesome service!!

Overall great app. We were able to incorporate a form on a landing page for applicants to provide details that help us determine if they are a fit for our program. Great option to customize a form to match the aesthetic and font styling so the page is cohesive. Customer support is responsive and helpful.

So far, so good. My only quibble is that the conditional logic is limited. For example, I'd like to be able to check a number field for greater than or less than conditions. I'd did a workaround, so no problem. Other than that, the app is excellent.

I contacted Hulk Form Builder assistance and within a couple minutes I received fantastic help by Adis with locating and editing a code on my Shopify account. Thank you very much for the quick fix on this matter!

I really like this app. We rarely have a need for people to register for things, so we just use the free version, but it's got enough features to get the job done!

Slightly buggy interface, miss-spelling everywhere, and the phone number dropdown form has very outdated country codes. Customer service rep was 10/10.
