I needed a form where people can upload files and this is perfect. It was easy to create and design the form how I needed it to be and implement onto my shop. Muaza from support on live chat was amazing and helped me when my form appeared on two pages and I couldn't work out how and why. Muaza fixed the problem there and then and explained what had happened - when you add your form as a block it will add to each page that uses that template. However, if you insert the short code in to the html it'll show on that page only.

I received exceptional quality customer support from Mirza. Not only was the support team very friendly, but they also helped me to resolve the issues I was facing in a timely and effective manner. Of the form apps I've tested, it offers the most value in the free plan. I'm considering switching to another plan soon to access more features. It has the best support I've ever had in any Shopify app!

Exactly what we needed to create embedded forms and easily review all answers! We had an issue with the integration on our theme, and it took a bit to get fixed, but the customer support was great and really helpful.
