Came across Hulk Form Builder recently and I have to agree it's one of the best app for building forms. I needed it on my career page so that people can upload there resumes. It solved the purpose. The forms fit in all screen sizes and works smoothly.

This app is just simply amazing. It has helped my business tremendously in terms of keeping my clients and their messages to me organized.
