BEWARE!!!! I've had this app for a year and it's worked fine but for some reason it has not been working right for the last month. I've had customers reach out to me directly saying they tried to contact me through the form and it didn't seem right. After testing it for myself, it is sending a confirmation email from a TOTALLY different email address that is not mine and NOT my shop. I have missed out on a month of inquires from my customers and this is pretty devastating to my business. BEWARE OF THIS APP!!!! I tried contacting customer support and didn't get a timely response or solution. I have no trust or faith left for this app and will be finding another one.

this app does not work at all. and they just let you pay for nothing. imagine making changes to your form and then the form just disappears ? it ridiculous how its built. not serious person will use it.

It's a little confusing at first, takes some getting used to. But I love that it's free and I can use it for many different things for my online store. Thank you for making this app for everyone here on Shopify!

I love this app. It is very easy to use and the support is fantastic and fast. We don't really have anything negative to say about it at all.

We've had a few issues here and there with the plugin, but the support is so excellent each time they've been resolved within days of reporting and the developers are very responsive. Other than that the plugin has some excellent features like dynamic fields that you can alter based on user input. highly recommended.

We use this app for our online forms and it's easy to set use. The tech support is helpful and quick, although the time zone difference delays things more than we'd like. We're in the US.
