Top app, does everything you want it to and more. The reason I'm writing the review though is for the excellent support, thank you!

Tricky to get onto storefront but team were helpful and efficient, Emma was who sorted it for me and I found them to be great!

The forms were very easy to to create and when I did have an issue, Emma from customer service fixed my problem quickly and professionally! A HUGE bonus to this app is that it's free.

Very good, but wish I didn't have to login to the app to see submissions. Otherwise, its a good app that I would recommend.
