Powerful Contact Form Builder

Lost my work several times due to crashes. I hate wasting time on an app like this. Time to start over. A couple of the issues:
1. One of the templates was completely broken upon loading it for the first time. Never got it to work
2. After failing to get that to work I built my own form and then it crashed when I tried to test it (clicked a check box and then the submit button on the form)

Super easy to customize and to install! Exactly what I needed. No coding experience required and beautiful templates.

The app is real good. Thanks for making our work, smart and not hard.
We recommend the app for those who do not know very much about development.

I have used several Shopify Page Building app. This is the best one. It really simple and fast to design your first form. It also has duo, or triple column layouts, which I really like. Recommended for everyone needing a simple, intuitive, easy to get started form builder.

Beautiful, easy and fast! I could make my own custom contact form, but I needed one that could let me upload files.

Absolutely fanatastic app where the devs have really covered all bases and produced something actually:
1) Easy to use
2) Functional and not full of bugs One (of the few) apps on Shopify that actually delivers!

was fast and easy to setup form, this is a good plugin if you are need of a form for your shopify website.

Had no issues and it did what I needed it to do. Nice one!
