Have been using the Pify Form for the last 6-7 months and it has worked flawlessly. Not sure there's a better, more easy to use form builder out there.

Seems great so far although I haven't really been using it long enough to leave an honest feedback but in order to remove the branding, I was asked to leave a review. Will update the review in a week or two
