Great plugin, love how easy it is to integrate with Mailchimp and Google Sheets, lots of customization!

I am using this app to collect submissions for my fundraising program, The Lu And Ed Lunch Program, to collect nominations to donate proceeds to. I will also be using it to create polls to vote for the next school from the submissions to donate to! Great app!
