This app is really easy to use and they have WONDERFUL support!
Richcarde has been super helpful on their support team! Two issues I have are: 1) I wish there was a bit more customization available, and I wish the form loaded faster on our website (it's always the last element to load on the page).

I used this app to help me build a form to take information for online personal training clients. I used the free version and it had everything I needed. Highly recommend.
It seems like it could be a great app, but there are several apparent bugs that make me uneasy about how well this app will actually work. The autoresponder feature seems neat, but I've tried sending a test email to multiple email addresses and have yet to receive the actual test email. So I filled out the form I created (which should trigger this autoresponder email), and I STILL didn't get the email! This is pretty frustrating.
Additionally, while creating the form, I had to refresh my browser multiple times while editing text sizes. I am not sure why this kept occurring, but it was extremely frustrating to have to refresh constantly to do such a simple thing. My web browser is up to date and this app is the only one I'm having issues with. Hopefully the developer can assist with a solution so I don't have to search for another functional form builder, again...

Problem: The only poor thing is that it loads a bit slow on my page, but that is maybe because I have the PRO version + many questions and options due to selling customizeable products.
Otherwise, the app really helps me build forms to retreive valuable information from my visitors.
Its flexible and works well.
Would not suggest due to a big issue we had getting the forms that were submitted.
It works pretty good! I prefer having this to the regular contact form as you can put your branded colors and ask for info you find important.
The DOWNSIDE of this is that it takes longer load then the original contact form.
Simple, get simple API links and all you need for Shopify Forms. I haven't looked around for a form solution for a while but this has matched and exceeded my expectations.

One of the easiest apps we have used so far. We were trying to create a custom survey for customers and this app allowed us to do that very easily. Best free app on the app store. Definitely recommended
