Amazing app. Best and simple form you can use with lots of integrations. Way better than Jotform or any of those apps with complicated forms..
Had some problems where the form didn't send through some information to Klaviyo to segments my leads. Support did an incredible and fast change to the whole app and now it works perfectly. Thanks!

A fantastic app, with a sync from the contact form to the backend email notification on the product queried. It was something that other apps (we tried about 5) don't have and you'd assume is an obvious feature. Props to Duke as well for his help solving an issue promptly. Highly recommend this app for anyone who needs an intuitive app, great support and features you would really need.

The form builder is a breeze to use. The ability to use conditional logic to show or hide various items on the form based on user input is also great, as is multi-step form options.
The form has so many options for customization built in, as well as the ability to use custom CSS, so it can fit into nearly any site design.
And, if you need help, the developer is also very responsive and quick to reply.

App is buggy as all get out. Drop down selections mysteriously disappear, icons shift on the page, and submissions get lost. Have to look for an alternative now.
I guess there was a misunderstanding with our app.
We just upgraded the app to meet Shopify's requirements.
So we've moved the app menu inside the app to Shopify's left menu, you probably didn't notice that.
You can still view everything as usual from our app.
We would appreciate if you could take the time to review this.

The software itself and the team's service are really great, but unfortunately, it's not compatible when accessed on a mobile device. I tried testing the Survey Form in the DEMO on my phone, but it couldn't display properly. It's such a shame.
