The form builder is a breeze to use. The ability to use conditional logic to show or hide various items on the form based on user input is also great, as is multi-step form options.

The form has so many options for customization built in, as well as the ability to use custom CSS, so it can fit into nearly any site design.

And, if you need help, the developer is also very responsive and quick to reply.

This app has no hassle to implement and its report is also clearly visible. Additionally, the support team quickly helps when we're in need (Thank you, Daniel!). I'm glad to find this app.

Great app, does exactly what I need. It had a small glitch when I first installed it and the tech team were great and helped me resolve it very quickly. Excellent contact form with great customization. We needed a form that would notify various email addresses based on the choice made by the user. There was a minor glitch upon initial install, but Duke was responsive and quickly helped with the technical issues. Price is great for the features it provides. Highly recommend!
