Best form creator with the most options and flexibility I could find in the app store. The only 2 things I would like to see added is the ability to set/change (and reduce) the line spacing between fields, and also the added option to place the labels to the left of the field instead of above or inside the field. Also support is very quick to respond and help. Thank you!

This app is exactly what I needed to add a custom form to my website. I love how user friendly it is and that it's fully customizable allowing me to achieve the look I wanted. Excellent support as well. Highly recommend!

I wanted to build a form for my customers to upload pictures for potential commissions, and add information that I would be asking them in the initial response anyway, and Form Builder with File Upload was perfect. Easily customizable too, so it doesn't stick out like a sore thumb on my page, even with my limited back-end design experience. Two non-sore thumbs up!
