Works well and is easy to set up. Allows image upload for your form. Excellent customer support as well. Recommend it for all stores!

Love it! I needed a form where users can upload images along with few information. This is the only app that gives image upload facility in absolutely "Free". Even though it is free app, they don't write any cheap label like "Powered by xxx" thats really cool.
