I used this app to build a customp form. Works great in my case to allow people to contact me on my site.

This is a great app. I had a little problem installing it, but Sarah at customer support was able to correct the problem right away.

i had an issue with latin extended characters not shown in customer mails. customer service resolved it very fast.

Great support team who is persistent and won't stop until a problem is resolved.
