Add custom fields on the cart page. Request the information you need to fulfill orders.
With our app, you can easily add custom fields to your cart page, and optionally make them required. Customers won't be able to visit the checkout area unless they fill out the information you need. The captured information is saved with Shopify and available to you in the order page. It can also be exported, displayed in emails and order printouts etc. The app supports the newest theme format known as Online Store 2.0.
- Field types: Text, Number, Checkbox, Radio, Dropdown, Combobox, and Text area.
- Simple drag and drop install process, built into the Theme Editor.
- Field data is saved in Shopify and easily accessible on the order page.
- Efficient code served from Shopify's servers — minimal impact on store speed.

We use this app for a order note that's for the customer (not for us as the merchant). It helps our customers if they can add PO#'s and job names or any note to their orders if they need to look back and reference anything. We then have this cart note displayed in all transactional emails using code. They also see the order note in the past orders page. It works flawlessly and we've been using this app for a while!
Does what it says, does it well and inexpensively, to boot!
The dev has been super helpful and responsive in guiding me through integrating the fields into pages in my theme!
