This is exactly the app I was looking for. Its simple to create a form and there are no locked features to make you buy into the subscription. The customer support is awesome and fast. Thanks to the whole team for providing such a great app!

Recently setup to be used as a Returns Form and works great with easy implementation into both the shopify store and our current internal RMA process. Considering it is Free I can happily say it is the ideal option for Small to Medium Business where a quick form is all you need.
