Crafty Order Fields

Easy to set up. It even has a multiple choice option where the customer can select more than one field. We only need one form at the moment, so free is a great price. Not sure why more people aren't using or reviewing this app. The previous reviewer that gave it a one-star was pretty harsh. I haven't needed support, but when it comes to the Shopify app store, I typically don't expect it for a free or low cost app. Great work!
