OSOM Post Purchase Surveys

Edit: Looks like this app is no longer supported. Support haven't been online in nearly 2 months, and my changes won't reflect on my site. So disappointed - Should have given your users a heads up at least.

Original: This is a great app - I’ve had 26 customers fill out the post-order question so far! It’s just a question asking how they discovered me and it’s actually great to know these answers!
