MAPMIGO: Store Locator & Maps

The app was exactly what I was looking for. It was very easy to add my locations and customize the store list and map. I had a change I wanted to make and the developer responded very quickly and gave me the code to make it work just as I wanted. On top of all that, it is free, so you can't go wrong!
