I have been using this contact form builder for years now. It's fairly easy to set up and customize options. It has a clean look and is easy to add to website pages with the automatic or manual option. Highly recommend!
Excellent Support & Reliable App
We’ve been using Nerdy Form Builder for our store and it has worked very well. Recently, we faced an issue with the sender name not displaying correctly when using a custom SMTP. I reached out to Anton (the founder), and he provided outstanding support. He quickly made updates to the app so that our emails now display the correct sender name, which is very important for our branding and customer trust.
The app is simple, effective, and the support is fast and personal. Highly recommended for anyone looking for a dependable form builder on Shopify.
Very easy and efficient
This form works great with my starter plan and I can customize it with a little design to make it stand out to customers whenever they visit the website. Simple, easy to use and you get a lot of access to many features even on the free plan. If you've got a starter store and you're just starting to buildup clientele, this is a useful app to help capture customer data. It actually works with starter shops.

Couldn't be more pleased with Nerdy Form Builder! Installation was a breeze and the app itself is very user-friendly, making it simple to create a form tailored to our needs.
Their support is also outstanding. Anton was super responsive to address any questions I had promptly. Overall, I highly recommend this app to anyone in need of a reliable form builder.

Do not use this app. The app has broken multiple times causing major issues for our business. They are unable to help solve the issues.
This problem was caused by a malicious user exploiting our application.
Which led to the app being blacklisted by security vendors.
It caused the app to be unavailable for some of our users.
Unfortunately, I wasn't able to find out the reason and solve it fast enough.
The application's security measures were improved, and this problem no longer exists.
You are always welcome to give Nerdy Form another try.
