The option appears on your cart, product pages and in checkout, always unchecked, so customers opt in themselves. You set the fee by cart subtotal and keep all of it. If something goes wrong, the customer reports it from their order status page. You see their note and photos, message them back, and approve a refund or send a replacement draft order. Every decision is yours.
