After a few months with this installed , had a customer messaging me asking why i have not responded with their form upload request. Checked the app and saw that even though its enabled, its still asking to be enabled. Worked once when I installed after received no more submissions. Uninstalling as this not reliable.

Thank you for your feedback. After checking, we found that you never submitted any support ticket. Based on our investigation, you changed the theme, which caused the app to be disabled. To prevent this issue, please enable the app whenever you change or update the theme.
