I spent two days trying to work out how to get this app up and running on my store. What a waste of my precious time. I needed to create different forms for different products. While this plugin has the configuration capability to do this, it actually doesn't work. So I gave up. Also the instructions are written in poor English and can't be followed. Considering the amount they charge per month, I am sure they could hire an English copywriter on Fiverr to improve the user guide.

This app is the ultimate tool with a very easy user interface. It allows for the access points we need to collect quotes online and streamline the process. But above all, the support is excellent. Each time we have asked for support or needed a feature customized to our needs, the support team is prompt and delivers on what they say they will. The customer support team were friendly and worked hard to resolve our problem - when the problem initially started we did have to wait 14+ hours for the support team to be in which could just be a time zone issue. Still would recommend the app if you are looking for a service like this!
