SureCust Forms Builder, Locks

My preferred app for this purpose. I've been using this app for over a year on one wholesale site, and just installed it on a second one.

I had a bit of trouble on my second install, and their support was fast and responsive. They diagnosed a compatibility issue with a second app we were no longer using within a day, and we were up and running.
