I'm using this app to create a form. Required some customization on the existing form fields, but that was not part of the app. Developer created fields for me and modified the app as per requirements. Support is quick and respond back positively.

This app is pretty good! It allowed me to build a beautiful custom form easily, and the Klaviyo integration is super helpful.
The only drawback is that you can't customize the template of the customer thank you email after they submit and inquiry- you can only change the text.
Customer support is very responsive as well.
I looked at several "form builder" apps on the store. This one does everything I need it to do and it's so simple to work with. I can try things out and then tweak them instead of redoing all my previous work. It's been great so far!
