I've been using the Hulk form builder app for some time now and I'm really happy with it. It's easy to use and customize, and it has a lot of features that I need. I can create different types of forms. Also, their customer support is top-notch, They solve our issue ASAP. I have had this for a few months and found it very easy to set up and use, although only have the free version at the moment and have had no problems.
