Very Good App
I began utilizing this application; it's incredibly simple to install and integrate into any desired webpage. It's the most impressive form builder I've ever experienced.
An amazing experience with the app and the customer service, even the pro plans have much more to offer for the new start-up, it has taken a lot of heavy load off my shoulders. I look forward to changing to Pro plans. I will surely recommend it to all New start-up businesses as well as established businesses.
UPDATE from ~6 months in: Still 5/5 stars. I've reached out to support for the odd question here and there, and every time I've gotten a response within *minutes*. And it's not just an auto-responder. It's a knowledgable, helpful person every time. Can't say enough good things about this app.
Original review: I've just set up my first form with the Hulk Forms app, and I have to say it's awesome! I tried it as I needed a form where my customers could upload images (how is that not a built-in feature of Shopify?!) and I've been blown away with the level of options. Whatever your use case, this app can definitely handle (but you may need to be at a paid plan for some of the customizations you need).
The only feedback I have is: I wish you could make a section conditional on multiple options (so if they selected option 1, 2, or 4, show the image upload button). Luckily you can easily create multiple conditional sections, so it's easy to accomplish, but it would help keep the forms as clean as possible - rather than having duplicate sections.

I used this app to create a "Get Involved" form. It's free, and it works great. My favourite feature is that you can have some questions only appear on the form if the visitor selects a specific option from a drop down menu. Also, you can have an automatic response sent to the customer from your email address when they fill out the form - so they know that the form went through. No complaints here - great options for a free app.
