The Form Builder works well, but the service that I received from HulkApps was even better. I had a tracking code that I needed to put into the form and they made it work!

This app is so easy to use and the support is great. We used the forms to connect different people to different services. The automation works great so that the requests are directed to the correct department for follow up. We also used it for privacy policy agreements, which was a lifesaver. Highly recommended.

Best Shopify Form Builder with Conditions and Hidden Field.
I bumped into form builder app when I was looking for custom work for my feast box menu. I wanted some fields to appear only if a condition was true and then I used their Advanced CSS block to get some details about my menu items within a hidden field.
Tip for consumers: once you try it, you will be impressed on how many integrations they have!!!!
