I used Form Builder to create a Wholesale Application form for our store. It works flawlessly. Was really straightforward setting up all the form entry fields and text, and most important has reCAPTCHA feature which sad to say is now needed for every form on any website.
I highly recommend this app based on my limited usage. I am using it in Free mode as that is all i need. I haven't looked at Pro mode.
A side note. I wish Shopify had reCAPTCHA for the "register" page and other form pages in a theme. It's crazy that Shopify does not build reCAPTCHA into Liquid so Themes can use it to add to forms. Come on Shopify! You can do it!

App works really well . Support is good too . easy to setup and configure . we needed an app which could allow us to make a option in form where customer can upload files .we found this app and it did the job for us .

Form Builder is simple and intuitive and I like that there is a free option, but can upgrade for more features if desired.
I needed more support for Volume & Tiered Discounts, but the Support Crew worked with me until I got exactly what I wanted - for free.

Excellent app, made it easy for us to create a form which enables customers to upload pictures of the used golf clubs they wish to trade in with us. What's even better is that the free version gets you very far.
