we used form builder to set up a wholesale page form. app works well and was super easy to use + install. great job!
It's sweet! It has really helped us form our ambassador program. Getting sales a few days after downloading the app. I just upgraded to premium however it still directs me to upgrade after I want to use some premium features... I selected the shopify payment option, so not sure if it is simply going to take a while to process, but it would be nice to have immediately.

Great app! Easy to use and would recommend it to everyone. Easy to customize and many great functions such as connecting to Google Sheet.
This is an awesome app. I had an issue today which Richcarde resolved very quickly. I was also happy to see that it is now Shopify 2.0 compatible which makes adding it to your site even easier than it used to be.
