Super easy to use - made a form in less than 10 minutes.
Don't love that support is only via email and seems to be on European timing. Otherwise, great!
Useful so far, videos online have been helpful. Their FAQ's responded to many of our questions and their website has all of the infromation you need.

Very easy to use. Easy to create nice forms and link them to other services like Mailchimp. Unfortunately the font selection is limited to Google's awful webfont collection, but that's a minor issue.
The app seemed to work well, easy to make forms, will continue to use and hope it works well for our business.

After looking through the App Store for a survey app, Form Builder was the easiest solution compared to the others I installed and tried out. It had all of the functionality that I needed and made building my form so simple, with great customization abilities. If this app could get any better, I would prefer to pay a one time charge per form rather then being billed ongoing for hosting a survey ...
I used the app to collect registration information at the time of paying fees. I really like how the forms data can automatically sync to Google Docs.
This app was very easy to edit and customize forms. It's cool that you can add prices to forms and edit confirmation emails to costumers!
I used this app to create an application form on my Shopify store. It is the only one I found that had the conditional logic I needed. It's fairly easy to use and did what I needed. Thanks!
