This app is great! I needed a form so I could get information from customers for a custom product. It was really easy to setup and get up and running. It integrated seamlessly into the product page with the automatic install.
I especially loved that I could customize an email for the customer to receive after submission, that was a really great touch. You can see my form in action here if you want to get an idea of how it looks on a product page.

Very easy to install, setup and make the forms, and the reporting is exactly what we were looking for. Nothing too complicated, but powerful enough for our somewhat difficult quoting scenarios.
Wow this is such a great tool. My website deals with custom orders all day long so its so easy to just have customers upload a picture and write exactly what they want in their own timing.
It was so easy to use, just download and follow the easy and simple steps. In less than 5 minutes I had my contact form up and running on my site.
