Pify Contact Form Builder

We used the Pify Form on our shop, and are very happy with how the form works. All we needed was a simple form with radio buttons, text boxes and dropdown selections. This app has provided it all and more! Customer service is excellent too. Got a very quick respond after sending an email and the issue was solved in a matter of minutes. Would definitely recommend it.
